This achievement is earned by beating King Dice (And all of the Mini-Bosses leading to him) without taking a single hit. Cuphead is a classic run 'n' gun set in the style of a one-on-one fighting game universe. I had gone through all the bosses without getting hit and collected one healthup when facing king dice.
